A. General Requirements

  • Police Investigation Report and/or Incident Report; and
  • Photographs depicting the damages sustained.

B. For Building Claim

  • A Civil Engineer’s or Architect detailed estimate for the restoration and/or repair of the damaged building;
  • Copy of the building plan and/or sketch complete with dimensions/specifications; and,
  • Declaration of real property.

C. For Stocks Claim

  • Formal computation of claim or a detailed inventory of articles damaged or destroyed showing the cost of each and extent of loss;
  • Inventory of affected and unaffected stocks as of date of loss, to include quantity and cost;
  • Documents in support of the unit cost such as purchase invoices or receipts and delivery receipts;
  • Copies of Income Tax Returns and audited financial statements for the last three years;
  • Latest Merchandise inventory filed with the Bureau of Internal Revenue.

D. For Machinery/ Equipment/ Furnitures/ Fixtures/ Fittings Claim

  • A complete description/specification of the affected and unaffected items as of the date of the loss showing its make and model, usage and year acquired, specific function and replacement cost supported by purchase invoices;
  • A written report/technical report as to the cause and extent of damage property and estimated value of salvage; and,
  • A formal statement of claim supported by quotations for replacement/repair for the damages sustained if repairable.
